Silverton Service Integration Team

The Silverton Service Integration Team is a collaboration of community partners that provide coordinated resources and information for individuals and families. We are comprised of agencies and community partners working to further improve the overall health and well-being of Silverton area residents by matching resources to clearly defined needs, while avoiding duplication of services.

Who We Are

Service Integration is a service delivery model to connect individuals and families to valuable resources and services. Service Integration uses an asset and solution-based approach to assist community members.

Silverton Service Integration Team is a collaboration between local health agencies and providers, local school districts, city/county/state agencies, local businesses, non-profit agencies, faith communities, local volunteers and leaders and other interested parties. If you are interested in collaborating with us on the Silverton Service Integration Team, we welcome you to attend one of our monthly meetings!

About Us
